Premise offers a wide range of dynamic, purpose-driven career opportunities. We are looking for a Full Time, Remote Case Manager RN to join our Occupational Health Team.   This position is 32 hours per week, no set schedule.  Excellent work/life balance! 

 

About the Role: 

  • This is a Remote role. It is Fulltime, 32 hours per week. Hours will vary.
  • New Jersey RN License is required to start.
  • Additional RN licenses will be required once hired; allowance will be provided.
  • Occupational Health. OSHA and Case Management experience is strongly preferred.
  • Provides clinical, casework, and group work services by performing the duties listed below.

 

ESSENTIAL FUNCTIONS 

  • Assesses the coordination and access to needed medical, psychiatric, social, educational, and other services essential for meeting the health needs of the individual
  • Coordinates assessment, treatment planning, and return to work processes with the employee, providers, and employer
  • Assesses and evaluates cases and prepares and implements treatment and case management plans using benchmarking to guide practices and timelines
  • Coordinates and provides care that is safe, timely, effective, efficient, equitable, and patient-centered
  • Counsels clients, assisting them to identify and resolve problems and make effective use of resources
  • Maintains collaborative working relationships with other community providers to develop specialized services as required
  • Acts as liaison between client employees, providers, and agency or institution
  • Assists clients in finding ways of adjusting to changes in physical capabilities that will allow for a life/work style as comfortable as the situation will permit.
  • Promotes quality and cost-effective interventions and outcomes
  • Provides consultation to staff on specific cases
  • Prepares and maintains written case records, reports, and forms, performs case follow-up and closing, and performs other administrative tasks as required
  • May perform duties telephonically as well as conducting on-site visits
  • May require other duties as assigned 

JOB REQUIREMENTS: 

Education: 

  • Certificate from an accredited Nursing School or Associate Degree (A.S.) required; Bachelor's Degree (BSN) from four-year college or university preferred.
  • Currently licensed Registered Nurse in state of practice required
  • Current hands on training in AHA or ARC Basic Life Support for health care providers is required. Advanced Cardiac Life Support may also be required based on contract scope of services.
  • Case Management (CCM) Certification, AANC – CM, American Academy of Case Management (AACM), COHN-S/CM, Certification in Occupational Health Nursing, Certification in Emergency Nursing, or Certificate in Case Management preferred. 

Experience: 

  • At least three years’ experience as a Registered Nurse in an emergency room, occupational health clinic or other ambulatory care clinic.

While a number of factors influence salary, our estimated California, Colorado, and Washington compensation is $80,000 - $100,000.
