The schedule is Monday - Friday, 7:30am-4:30pm (1 hour lunch). NO Nights! NO Weekends! NO Holidays!

About the Role RN position that leverages powerful data insights to proactively identify and engage members who need support the most. Nurses will design individualized care plans, offer self-management education and coordination of resources that assists members to achieve improved health. Collaborates closely with clinical teams to provide guidance on engagement strategies, focused on improving health status of members. Manages a panel of high-risk members with chronic conditions to provide frequent touchpoints and ensure care gap closure as defined by national standards. Assists the clinical team with coordinating care for high-risk members.
The Wellness Coach is a certified coaching professional who specializes in behavior change theory and process. The Wellness Coach assists members in co-creating a personalized wellness vision to achieve optimal health and wellbeing.
ESSENTIAL FUNCTIONS
- Develops personalized treatment plans using provider care plans and member preferences using a coach approach and the pillars of Lifestyle Medicine
- Supports a proactive, multidisciplinary team approach directed toward prevention, education, and health promotion
- Provides condition specific self-care education to members enrolled in the various condition management programs
- Participates in outreach/follow-up phone calls and follow-up member visits
- Creates documentation using electronic systems (i.e., EMR, charting or other specified program etc.) for each interaction
- Collaborates with health center staff to provide optimal member care and engagement
- Strives to work with member to improve health outcomes and better management of any chronic condition
- Works closely with client’s vendors as appropriate to support member’s needs where appropriate
- Identifies emergency situations and provides care within legal scope of practice
- Acts as facilitator of Care Team meetings.
- Delivers one on one and/or group coaching sessions in the areas of weight, nutrition, physical activity, stress, tobacco cessation, sleep, work/life balance and general health
- Creates a supportive environment for positive lifestyle changes
- Partners with members to assess their current level of well-being
- Educates member on various aspects of health and wellness, providing evidence-based information and resources to support their progress.
- May coordinate and conduct wellness programs in areas such as nutrition, stress management, weight management, tobacco cessation, physical activity and others
- Works closely with third party health and wellness vendors
- Presents in an effective and professional manner.
- May interact directly with the client or client representative
- May require other duties as assigned
Job Requirements:
Education:
- Currently licensed Registered Nurse in state of practice
- National Board Certified Health and Wellness Coach (preferred)
- Certified by a National Board Certified Health and Wellness Coach (NBC-HWC) approved program or ability to complete an approved certification within 24 months of hire (Wellcoaches preferred)
- Current hands on training in AHA or ARC Basic Life Support for health care providers required; Advanced Cardiac Life Support may also be required based on contract scope of service.
- Bachelor's degree (BSN) from four-year college or university (preferred)
Experience:
- At least three years’ experience as a Registered Nurse caring for members with any of the following conditions: Type 2 diabetes, asthma, heart disease, heart failure or hypertension.
- Experience working as a wellness coach
- Experience in Condition Management, Certificate in Case Management, Care Coordination, Certified Diabetes Educator (CDE) Certified Diabetes Care and Education Specialist (CDCES) a plus (preferred)
- Experience in health promotion or work site wellness and public speaking (preferred)
- Experience with Electronic Medical Records (preferred)

For individuals living in California only, Premise Health is required to include an estimate of the salary and benefits for this role. While a number of factors influence salary, our estimated California compensation is $103,500 - $115,000.
