As a Full Time Registered Nurse, Care Manager, you'll provide care to client employees and their dependents in our Health Center located in Plano, TX. The scheduled hours are Monday - Friday, 8:30a - 5:00p.

What You’ll Do

  • Conducting in-person visits to assess patient health status, set goals, and provide education on weight management strategies.
  • Delivering motivational interviewing and behavioral coaching during face-to-face encounters to support lifestyle changes.
  • Review member facing educational materials with participants regularly to help them build a strong foundation of Lifestyle Medicine practices.
  • Coordinating follow-up visits and outreach to maintain continuity of care, address medication concerns related to AOM therapy, and encourage adherence to prescribed medications.
  • Documenting all patient interactions, assessments, and care plans in Epic to ensure accurate and timely record-keeping.
  • Using Epic tools to track progress, flag non-compliance, and communicate with interdisciplinary team members.
  • Setting reminders and alerts in Epic for preventive screenings, lab follow-ups, and medication adherence.
  • Performing regular panel reviews to identify patients overdue for visits, labs, or screenings related to obesity and comorbid conditions.
  • Reviewing dashboard reports from Epic to monitor clinical outcomes, address care gaps, and compliance with evidence-based guidelines.
  • Engaging in quality improvement initiatives by analyzing panel data to drive accountability and improve population health metrics.


What You’ll Bring

  • Currently licensed Registered Nurse in state(s) of practice
  • National Board-Certified Health and Wellness Coach (preferred)
  • Certified by a National Board-Certified Health and Wellness Coach (NBC-HWC) approved program or ability to complete an approved certification within 24 months of hire (preferred)
  • Current hands-on training in AHA or ARC Basic Life Support for health care providers required; Advanced Cardiac Life Support may also be required based on contract scope of service.
  • Bachelor's degree (BSN) from four-year college or university (preferred)
  • At least three years’ experience as a Registered Nurse caring for and coaching members with chronic conditions
  • Experience working as a wellness or health coach
  • Experience in Condition Management, Certificate in Case Management, Care Coordination, Certified Diabetes Educator (CDE) Certified Diabetes Care and Education Specialist (CDCES) a plus (preferred)
  • Experience in health promotion or work site wellness and public speaking (preferred)
  • Experience with Electronic Medical Records
