Responsibilities


Premise Health is currently seeking a Primary Care Physician to work remotely full time for our Virtual Assisted Primary Care Team. 

ESSENTIAL DUTIES AND RESPONSIBILITIES  

 

  • Utilizes advanced telehealth technology to provide primary care and oversight for the employer-based population by reviewing all health risk factors gained through a health risk assessment, physical and evaluation. 
  • Works in close partnership and supervises nurses who act as the onsite facilitators for acute and primary care visits. 
  • Conducts annual health risk assessments on members and adult dependents as needed and develops tailored action plans with patients. 
  • Serves as a resource for all ancillary professionals and eHealth nurses who provide support to patients using the virtual health center. 
  • Provides quality, evidence-based medical care that is routinely provided in a primary care setting for multiple clients and through multiple nurses utilizing technology to bridge geographic distance. 
  • Understands and implements medical protocols and clinical standards. 
  • Develops relationships with the community of providers in order to oversee all the clinical health of the members in a cohesive manner. 
  • Diagnoses and treats acute injuries and illnesses using technology-enabled equipment while engaging patients via telephonic and video modalities. 
  • Diagnoses and manages chronic diseases such as diabetes, hypertension, hyperlipidemia, and obesity. 
  • Serves as a consultant for difficult patient cases, adverse health trends, and safety concerns. 
  • Provides education regarding current occupational medical practices and regulations to nurses. 
  • Reviews and provides information regarding clinical questions raised by clients and patients. 
  • Recommends changes in all clinical areas and general areas of the business to improve patient health and wellness. 
  • Establishes goals for each patient to be monitored and tracked by support staff. 
  • Supports patients in changing health related behaviors. 
  • Participates in patient education sessions among targeted populations with prevalent health risk factors (e.g. high body mass index, smoking, diabetes). 
  • Provides telephonic care as needed to support telehealth services.

  • Adheres to all company policies, procedures and business ethics codes.  

QUALIFICATIONS 

 

  • Board certified without restriction Internal Medicine or Family Practice 
  • Valid, current, and unrestricted state license and DEA license 
  • Familiarity and experience working with technology-enabled equipment in a virtual environment 
  • Ability to engage patients through telephonic and video interactions in order to deliver acute, primary, and chronic care 
  • Ability to work as a member of a remote team of physicians delivering care to distant patients while demonstrating mutual respect for colleagues, clients, and all partners 
  • A commitment to working collaboratively with the member/patient, the eHealth nurses, and the wellness team to improve patient health through behavior change methods, including but not limited to, motivational interviewing 
  • Computer literacy, including knowledge of software applications and familiarity with electronic medical records, MS Office, internet, and mobile applications 
  • Keyboard and internet navigation skills, including the ability to use online applications to document accurately and concisely 
  • Good leadership skills and communication skills 
  • A commitment to continuously improve the health and wellness of each member 
  • A sense of collaboration and cooperation with a willingness to continually learn
